The application form is for students who wish to matriculate into a program at Paier College of Art.
The fee for the application is $25.00.
At this time, you will need to download and complete the application and mail the completed application to:

The Immunization Record Page is included in the application download and must be sent to your Physician or to the last school you recently attended. Their office will need to forward us your Immunization Records for College acceptance.
